NOTE: The instructions below apply only to accessing up to 5% of your OBS funds. If you would like to transfer funds from your non-registered OBS to your registered OBS, please visit your branch and ask to speak with a Financial Planner or Investment Clerk.

Instructions:

2. This form is a PDF which you can fill in electronically and print or print and fill in manually
3. Fill out all areas of the form and sign it. Do not complete the sections labelled “Internal Use Only”. These areas will be completed by MSCU staff.
4. Mail or bring the form to your Branch for processing on or before June 13, 2008 to have the transfer appear on your June statement or on or before November 14, 2008 to have the transfer appear on your December statement.
5, If you have access to a scanner and would like to scan the completed form and email it to MSCU, it can be sent to info@mscu.com.
6. If you have access to a fax machine and would like to fax the form to MSCU, contact your branch and request their fax number.
